NEXT MESSAGE: “If
you are a provider press 1...” You
will need: Your fax number, federal ID number,* and insured Social
Security number.
“If
you are insured and wish eligibility or claim status press 2...”
You will need: Your social security number, birth date, and date of
claim.

Participant’s Social Security Number

Number including Area Code (if requesting verification and a summary
of benefits or a copy of an Explanation of Benefits)

of Service (if requesting status of a specific claim)
Following
the initial greeting, as soon as you know which menu item you wish to
select, you may press the appropriate number without waiting for the
system to complete its explanation.
A
date of service must be entered in a six digit format; for example,
January 5, 1999 would be entered as 0-1-0-5-9-9
